Two Underworld enemies drop it, which makes it a byproduct of any long trip down there rather than something you set out to farm. Bone serpents are harmless once you have room to move. Red devils are the problem, since the trident they throw tracks you and takes a serious bite out of an unprepared character. A platform with a few walls to break line of sight makes the collection much calmer.

It grants the strongest food buff, so it is a pre-boss item rather than something to burn topping off a weaker one. Since hell is where you go before the Wall of Flesh anyway, a hellstone mining run doubles as a food run for the fights that follow.
