what it actually changes
Treat it as a travel item and you will undersell it badly. The real use is a panic button with no wind up: one click and you are somewhere else, before the attack that was about to land arrives. That covers things the game otherwise expects you to eat, the Moon Lord deathray, a Dungeon Guardian closing on you, half of what the Empress of Light throws out at full speed. Fights that felt like a damage race turn into a positioning problem. It also makes catching Truffle Worms far less fiddly, since you can drop onto one before it burrows.
the cost of a second jump
Every use leaves Chaos State on you for a few seconds. Jump again before it clears and you pay a fixed fraction of your maximum health, and that payment is unusually hard to avoid. Nothing reduces it. No dodge accessory eats it. It passes straight through invincibility frames without granting any, so it can land in the middle of a hit you were already taking. Journey mode godmode is the only thing that stops it. In practice this means teaching your hands a rhythm: click, count, click. Players die here by mashing the button in a panic, not by misjudging arithmetic.
aiming it
Your feet land on the cursor, not your center, and that is the source of most failed jumps. To slip into a low opening you aim at its floor rather than its middle. The destination needs clearance as well, a couple of tiles wide and three tall, and while mounted it needs more than that because the game has to fit the mount in too. A unicorn wants a noticeably taller gap. A jump that fails plays the same noise as one that works but nothing happens and no debuff lands, so the sound is not confirmation. Reach is capped too, generously sideways and much less so vertically, and clicking past the edge of it does nothing.
the details that keep you alive
Momentum carries across. Jump while running or falling and you arrive still moving at the same speed, which is what makes chained hops across a boss arena work instead of stalling you in place each time. Falling damage is measured from wherever you land, so a jump partway down a shaft wipes the drop you had built up and lets you take the rest safely. Shroomite stealth survives a jump as well, so a ranged build can reposition without giving up the bonus it stood still to earn.
where it will not take you
There is a hard block on the Jungle Temple until Plantera is down, and the rule is written around the wall rather than the room: anywhere underground with unsafe Lihzahrd brick behind it is off limits, so you cannot cheat the door from any angle. Remix worlds extend that restriction above ground too. Dual Dungeons worlds add Unbreakable Walls, which the jump also refuses to cross. Everything else in the world is fair game, including places you have no other way of reaching.
getting one, and what it becomes
It falls from Chaos Elementals in the hallowed caverns, and those are available the minute a world turns, so one of the scarcest drops in Terraria sits behind almost no progression at all. In practice it either lands early enough to reshape a whole playthrough or never turns up, and farming for it deliberately is an afternoon of work. The Celebration Mk 10 seed makes it far more common at any difficulty. Much later, dunking it in Shimmer once the Moon Lord is dead upgrades it into the Rod of Harmony, which is the same effect without the health tax.
using the damage on purpose
Because the health loss is precise and on demand, this is the cleanest way to trigger accessories that reward being hurt. A Shrimpy Truffle or a Panic Necklace can be switched on whenever you want the speed rather than whenever something manages to hit you, and the flight achievement stops being a matter of luck. Dying to your own teleport gets its own death messages, which is the game quietly acknowledging how many people have managed it.
