Not a real item. It exists in the files with a tooltip attached and nothing behind it: no attack, no animation, no behavior of any kind, and no route to obtaining it in normal play.
The interesting part is what sits next to it. There is an unused projectile in the code with a shouted, wordless name that shares this item's internal identifier, and that identifier points at some kind of sound-based gun that was sketched out and dropped.
It also behaves oddly compared to other unreachable content. The game normally strips items like this out of an inventory the next time the character loads. This one survives that check and stays where you put it.
