It is a single eyeball mounted on a coiled tentacle that hops along behind you. Get far enough ahead of it, or jump, and it spins the tentacle like a rotor to close the gap, which is the one bit of animation anybody remembers. It does nothing in combat and it occupies your pet slot.
where it comes from
Eyezors drop it, and they live in the Hardmode dungeon, so this is something you pick up while farming that floor for gear worth having. Expert worlds hand it over a bit more often.

- The eyeball is the Eyezor's own, pried loose and given a spring to bounce on.
