Pure vanity. Walking around trails small sparkles behind your feet, there is no stat attached, and it cannot be reforged, so no modifier will ever sit on it. Keep it in a social slot rather than a working accessory slot. The sparkle shows either way and you get to spend the real slot on something that does something. The Princess is the only source, and she is among the last town NPCs you can bring in, so this arrives very late in a run no matter how you play.
where the name comes from
It is Cinderella's slipper, the one left behind on the stairs. The tale itself is centuries old and retold everywhere, but the sparkle trail points at the 1950 animated film rather than the printed version.
