the heal is the point
Every swing that connects gives a small amount of health back. On a single target that trickle is meaningless. Swinging into a pack, where one arc catches several enemies at once, it turns into steady regeneration that can outpace the chip damage you are taking. Pre-hardmode has very little life steal in it, which is what makes this worth a slot even when a stronger sword is available.
the drop
Any bat anywhere can drop it, and the odds per bat are low enough that you should not build a farm for it. It arrives on its own while you are mining. On The Constant seed the chance is considerably better, which fits, since the tooltip is a nod to the game that seed is imitating.
