Swing and a leaf spirals up before curling back toward whatever is nearby. There is no aiming it in any meaningful sense. It only picks up targets fairly close to you, so treat it as a bonus against things already in your face rather than a way to reach across a room. It does pass through terrain, which makes it useful for poking at whatever is under the floor you are digging. Lava destroys the leaves outright, so it adds nothing in the underworld.
poison is the real damage
Both the swing and the leaves can leave enemies poisoned, and against the health pools you meet at this point in a run that tick adds up faster than the hit that started it. It also means you can disengage after connecting and let the damage finish on its own. Against armored targets, the leaves cut through defense far better than the blade does.
why you keep it
This is one of the four swords that feed the Night's Edge, which is why most people craft it at all. The jungle materials are the easy part of that recipe next to hunting down the underworld and dungeon pieces, so grab this one early and let it sit in a chest until you have the rest.
