Normally letting go of the button reels the yoyo home and the damage stops. With this equipped, releasing flings it forward instead and it keeps hurting things on the way out. Your rhythm shifts from parking the yoyo on a target to throwing, releasing and rethrowing, which suits fights where you need to land a hit and immediately move.
how the loose projectile behaves
A thrown yoyo skips off floors and ceilings and rolls along flat ground, but a wall met side on kills it instantly. Corridors and arenas with a clean floor are where it earns its slot; broken terrain eats the throws.
Each one caroms between a handful of enemies before it gives out, so it is far better into a crowd than against a single boss. Counterweights get thrown too, but they pop on the first thing they touch.
the invincibility catch
Released yoyos apply the global flavor of invincibility frames. Every hit they land briefly locks the target out of taking damage from anything else, so on a build stacking minions, sentries or a second damage source this can quietly cost you more than the extra projectile adds. The Terrarian is the exception and does not have the problem, which is part of why it stays good with this on.
